This page explains what happens to the information you send through this website. It covers the website only — the clinic's Notice of Privacy Practices for your medical records is provided at your visit.
What we collect
The appointment request form collects your name, phone number, and optionally your email address, preferred date and time, whether you're a new or existing patient, where you'd like to be seen, your insurance type, and anything you write in the “how can we help?” box.
Please don't send medical details
The form is a request to be called back — not a medical intake. Email is not a secure channel, so we ask you not to include symptoms, diagnoses, medications or other health information. Our staff will go through all of that with you by phone or at your visit, where it is protected as part of your medical record.
Where it goes
Submissions are emailed directly to the clinic's front desk through our email delivery provider, and are used only to contact you about your request. We don't sell your information, and we don't use it for advertising.
Cookies and tracking
This site sets no advertising or tracking cookies. If you arrive by scanning our printed QR code, the link carries a tag that tells us the request came from print — nothing that identifies you.
Emergencies
This website is not monitored around the clock. If you have a medical emergency, call 911 or go to your nearest emergency room.
